Referrals

We accept patients by physician referral only. This allows our practice to share information with your physician about our findings and recommendations. It is critical for your physician to be aware of interventions performed in order to manage your condition after our evaluation.

Insurance

Our office accepts most insurance plans. If your plan requires a referral, please be sure to obtain the referral prior to your visit. We require your specialist co-pay at each visit. If you do not have insurance, payment is due in full at the time of your visit.

Appointments

New patients should complete patient registration and medical history forms ahead of time when possible. Please be sure to bring your driver’s license and insurance card(s) to your appointment. In order to make the most of the time you spend in our office and to avoid duplication of testing we require copies of any medical records related to the reason for your visit. At the time of appointment scheduling, make sure we are aware of all testing and hospitalizations so we may obtain the records prior to your appointment.

If you cannot make your appointment, we ask that you give us 24 hours notice.
This allows us to schedule a patient on our waiting list. There is a $75.00 fee for appointments missed without 24 hours notice.

After Hours Care

If you feel your problem cannot wait until the next business day, you can call our exchange at
314-388-6269 where you will be directed to MBMC’s Emergency Room. Our doctors are available after hours to the ER physicians for advice after the evaluation (exam, labs, radiology) has been completed. You must request OUR PHYSICIANS by name upon arrival to avoid duplication of services.

If you have a true medical emergency, you should contact 911 or go to the nearest emergency room.

Patient Services

Prescription Refills
Please contact your pharmacy for refills. They will notify us of your request. Please allow 24-48 hours for completion of refills. This allows for review and completion of your request. Please do not wait until you are out of medication to request a refill. Our office is unable to fill medications urgently.

FMLA and Disability forms
FMLA and Short-term Disability forms will be completed by our office. Be sure to include YOUR information including your name, and signature as required prior to leaving the forms at our office. We will complete them and fax to your employer. There is a fee for FMLA and Short-term Disability Forms up to $30.00.

Telephone Calls
Routine phone calls are taken between the hours of 8:30 am and 5:00 pm, Monday through Friday. We strive to respond to our patient needs promptly and completely. Please keep in mind that our advice requires chart reviews as well as clinical and laboratory evaluation. Return calls are made throughout the day and after office visits (after 5 pm). If your call is urgent please be sure to communicate this to the medical assistant. Please be sure to leave the phone number where you can best be contacted. Please do not leave multiple messages. (They delay service to other patients.)
We do not take calls after hours direct from patients due to the strictly referral nature of our practice.
